Free Pre-Diabetes Individual Appointments
Learn about the risk factors for diabetes and steps to manage pre-diabetes. Call (219) 325-7623 for an appointment. A physician order is not required.
Diabetes Self-Management Classes
This program empowers you to control your own health. A physician’s order and pre-registration are required. Classes begin at the start of each month and continue for four weeks. Classes are offered on Mondays from 9 a.m. to 11:30 a.m. or Thursdays from 5 p.m. to 7:30 p.m. Call (219) 325-7623.
Individual Appointments
A physician’s order and pre-registration are required. Appointments may be made with either a certified diabetes educator or registered dietitian, depending on your treatment method and needs: insulin, injectable, gestational diabetes, continuous glucose monitoring and nutritional education. Diabetes Support Group
The Diabetes Support Group provides a place to share ideas, experiences and problems related to diabetes for those with diabetes, as well as their support team. The group meets the fourth Monday of the month from 11:15 a.m. to Noon at Northwest Health - La Porte, 1331 State Street, Lilly Lake Conference Room (fourth floor). Registration is not required and the program is free.
