The diabetes educators and clinical dietitians at Northwest Health - La Porte are available to provide information about preventing and managing diabetes.
There are more than 30 million people in the United States with diabetes and that number is growing. Finding out you have diabetes can be scary and overwhelming. You can control your diabetes and live a long, healthy life when you know what steps to take to manage your disease. Learning as much as you can about diabetes and understanding the tools you need to care for yourself are the most important steps to being successful in managing your diabetes.
Northwest Health - La Porte is dedicated to improving your quality of life through care and education. The self-management program helps you learn the skills and knowledge needed to better manage the disease and avoid complications.
The diabetes and nutrition care team provides education for type 1, type 2 and gestational diabetes, prediabetes, hyperglycemia and hypoglycemia through a range of services, including:
- Certified diabetes care and education specialists who can assess and identify your needs to help create your game plan for diabetes
- Registered dietitians who provide assessment and identify needs to help you create an individual food plan
- Nutritional counseling, including meal planning and weight management assistance
- Diabetes self-management education/training classes, which are shown to help improve diabetes management
- Individual, customized sessions based on your needs
- Insulin instruction
- Continuous glucose monitoring
- Blood-sugar testing
- Diabetes education to promote healthy living
